LPN Medical Care Manager

About the role

The LPN Medication Nurse provides licensed nursing support in a senior living environment, ensuring residents receive safe and accurate medication administration, health monitoring, and proper documentation. This role is essential in safeguarding resident health through clinical precision, regulatory adherence, and compassionate care.

Responsibilities

• Administer medications and treatments following physician orders and the Six Rights of Medication Administration.
• Accurately document in MAR/TAR in real-time and transcribe physician orders.
• Monitor residents' symptoms, report adverse reactions, and observe physical, emotional, or cognitive changes.
• Maintain medication carts, complete narcotic counts, and ensure compliance with regulatory standards.
• Conduct Service & Health Updates (SHUs) and support care plan updates.
• Report resident health changes promptly to ensure safety and quality care.

Qualifications & Skills

• Active LPN license in good standing.
• CPR and First Aid certification.
• Strong assessment, documentation, and clinical skills.
• Preferred experience in senior care or medication administration.
