LPN Medical Care Manager

About the role

We are seeking an LPN Medication Nurse responsible for providing safe and accurate medication administration, health monitoring, and thorough clinical documentation in a senior living setting.

Responsibilities

• Administering medications and treatments following physician orders and the Six Rights of Medication Administration
• Documenting accurately in MAR/TAR in real time
• Monitoring residents for symptoms, adverse reactions, and physical, emotional, or cognitive changes
• Reporting refusals or adverse reactions promptly
• Supporting care plan updates and transcribing physician orders
• Maintaining medication carts, completing narcotic counts, and ensuring compliance with regulations

Qualifications and Skills

• Active LPN license in good standing
• CPR and First Aid certification
• Strong assessment and documentation skills
• Experience in senior care or medication administration preferred
