About the Role

The Licensed Practical Nurse (LPN) delivers professional nursing care to residents of all ages in a long-term care setting. Under supervision, the LPN assesses residents, administers medications and treatments, documents care activities, and responds to emergent situations. The role requires establishing effective relationships with residents, families, and healthcare team members while maintaining confidentiality and clear communication.

Responsibilities

  • Provide resident-centered nursing care aligned with care plans.
  • Evaluate resident status through testing and observation.
  • Recognize and respond to health problems, offering solutions and interventions.
  • Educate residents and families on prevention and self-care strategies.
  • Maintain accurate documentation of care, medication, and resident interactions.
  • Support interdisciplinary team efforts and contribute to a safe, comfortable environment.

Requirements

  • Graduate from an accredited practical nursing program; associate degree preferred.
  • Hold an unencumbered LPN license with multi-state privileges.
  • Possess IV therapy/management skills if required by the state.
  • Minimum of 400 hours clinical experience or supervised clinical training across the lifespan.
  • Complete ongoing competency and continuing education requirements.
