LPN - Home Health | Full-Time, Days

General Summary

The Licensed Practical Nurse (LPN) at Phelps Health assesses patients, implements nursing care plans, coordinates care with other disciplines, and documents care efficiently. This position requires a strong commitment to providing life-saving care and supporting patients.

Essential Duties And Responsibilities

• Evaluates patient eligibility for home health services based on CMS or payer guidelines and agency policies.
• Accurately assesses patient conditions, reporting changes promptly to Registered Nurses (RNs).
• Performs procedures and treatments according to established policies and infection control guidelines.
• Documents care within specified timeframes and organizes nursing care in a cost-efficient manner.
• Collaborates with other healthcare professionals to develop and implement discharge plans and patient/caregiver education programs.
• Evaluates the effectiveness of care plans and individualizes teaching methods based on patient needs.
• Provides feedback to RNs regarding patient progress and educational outcomes.
• Manages time effectively to meet daily productivity goals.
• Reflects on performance to identify areas for improvement.

Education and Experience

• Graduate of an accredited LPN program and approved IV therapy course.
• One year of home health care experience preferred.

Certification/License

• Missouri or compact state LPN license.
• American Heart Association Basic Life Support certification.
• Valid Driver’s license and proof of insurance.

Mental/Physical Requirements

• Considerable mental concentration required.
• Lifting up to 35 lbs., turning activities, and nearly constant walking are required.
• Standing, turning, carrying, pushing, pulling, stooping, crouching, twisting, and reaching are frequent.

Working Conditions

• Frequent exposure to communicable diseases via blood and body fluids.
• Frequent exposure to infectious diseases and hostile persons.
• Works with sharps and other potentially biohazardous materials.
• Variation in environmental temperatures.
• Exposure to various community environments.
