General Summary

The Licensed Practical Nurse (LPN) at Phelps Health assesses patients, implements nursing care plans, coordinates care with other disciplines, and participates in call rotations.

Essential Duties And Responsibilities

• Evaluates patient for continuing eligibility for home health services according to CMS or payer guidelines and agency policy.
• Accurately assesses patient conditions, reporting any changes promptly to an RN.
• Performs procedures and treatments as per policy and infection control guidelines.
• Documents care within specified time frames and organizes nursing care efficiently.
• Collaborates with other healthcare professionals and develops discharge and patient education plans.
• Assists in evaluating the effectiveness of care plans and individualizes teaching methods based on patient needs.
• Manages time effectively to meet daily productivity goals.
• Reflects on performance to turn mistakes and successes into learning opportunities.

Education and Experience

• Graduate of an accredited LPN program and approved IV therapy course.
• One year of home health care experience preferred.

Certification/License

• LPN license from Missouri or a compact state.
• American Heart Association Basic Life Support certification.
• Valid Driver’s license and proof of insurance.

Mental/Physical Requirements

• Considerable mental concentration required.
• Lifting up to 35 lbs., turning activities, and nearly constant walking are required.
• Standing, turning, carrying, pushing, pulling, stooping, crouching, twisting, and reaching are frequent.

Working Conditions

• Frequent exposure to communicable diseases via blood and body fluids.
• Frequent exposure to infectious diseases and hostile persons at times.
• Works with sharps and other potentially biohazardous materials.
• Variation in environmental temperatures.
• Exposure to various community environments.
