LPN - Home Health - Full Time

About the role

Provides skilled nursing services in patients' homes under the supervision of a Registered Nurse, assessing, planning, implementing, and evaluating care needs.

Responsibilities

• Manages a caseload of patients with diverse health conditions, delivering patient education, treatment, and emotional support to promote recovery and manage chronic illnesses.
• Maintains excellent communication with patients, families, and healthcare professionals, ensuring accurate documentation and updates on medication and care plans.
• Provide high-quality, patient-centered care in the community, fostering independence and meaningful health outcomes.
• Collaborate with healthcare teams and respond proactively to changes in patient conditions.

Requirements

• Licensed Practical Nurse (LPN) with current licensure.
• CPR certification.
• At least 1 year of experience.
• LPN-IV certification required or completed within 3 months of hire.
• Must be a licensed driver.
