About the role
The Health and Wellness Director (HWD) is an LPN who oversees and supervises the care of 64 residents at Aspen Hills Senior Living.
Responsibilities
- Oversight of health services department
- Ensuring resident care needs are met
- Promoting a positive physical and psychosocial environment
- Creating a resident-centered approach to services
- Accurate completion and use of level of care determination tools
- Assessing and evaluating residents
- Developing and implementing individualized service plans
- Managing care and services for residents
- Case managing and meeting the needs of residents
- Coordinating with other professionals and ancillary services
- Communicating with residents' families
- Providing education and support to residents and families
- Supervision and training of health services team members
- Managing delegation of nursing tasks
- Participating in investigations and root cause analysis
- Overseeing documentation and communication of resident care and services
- Maintaining infection control practices
- Providing on-call support and phone triage
- Participating in leadership team meetings
- Representing the community in a positive manner
Qualifications
- Graduation from an Accredited School of Nursing as an LPN
- Unencumbered license and good standing with the State Board of Nursing
- Required to have a current RN License
Benefits
An LPN can expect a salary range of $70,000-80,000, commensurate with experience. The Health and Wellness Director is full-time and as such, receives full-time benefits with PACS.
