About the role
The Health and Wellness Director (HWD) is an LPN who oversees and supervises the care of 87 residents at Autumn Wind Assisted Living in Caldwell, Idaho.
Responsibilities
- Oversight of the health services department and ensuring resident care needs are met by coordinating resident care, managing health services systems, and providing supervision to health services team members.
- Promotes a positive physical and psychosocial environment for the residents.
- Creates a resident-centered approach to the planning and delivery of services to residents, supporting residents’ rights to participate in decision-making and treating residents with dignity and respect.
- Accurate completion and use of level of care determination tools to ensure residents receive the care and services they require and desire.
- Assesses or evaluates residents with reported changes of condition.
- Provides team member direction, service planning, and coordination of care to meet the residents’ changing needs.
- Collaborates with the management team on the move in of prospective residents, taking into consideration the needs of the prospective resident, current community acuity, and staffing levels.
- Communicates regularly with residents’ families regarding health care needs.
- Provides education and support to residents and families.
- Supervision and training to health services team members including personal care and medication management.
- Manages the delegation of nursing tasks as allowed under applicable state nursing laws and practice acts.
- Partners in the investigation and root cause analysis of incidents.
- Collaborates with the management team on the development and implementation of appropriate interventions.
- Ensures resident records are completed in a timely manner and maintained according to company policies and state regulations.
- Assists in the management of infection control.
- Maintains up-to-date knowledge of all rules, regulations and policies pertaining to resident care.
- Stays informed of industry and health care trends and standards, applying new knowledge to meet the changing needs of residents.
- Provides on-call support and phone triage for team members’ questions regarding resident care.
- Participates in leadership team by actively contributing to decision making, incident follow up, community and company-wide initiatives and attending leadership team meetings such as daily stand up, clinical meeting, Quality Assurance Performance Improvement (QAPI), and other meetings as required.
- Acts as an ambassador by representing the community in a positive manner to outside agencies and participating in community events and social functions.
- Works within scope of license with RN oversight as needed.
Qualifications
- Graduation from an Accredited School of Nursing as an Licensed Practical Nurses (LPNs).
- Must hold an unencumbered license and be in good standing with the State Board of Nursing and maintain all required continuing education/licensing requirements at all times.
- Nurse Management Experience preferred.
Skills
- Microsoft Suite products proficiency.
- Point Click Care (PCC) proficiency.
- Current RN License.
Benefits
An LPN can expect a salary range of $83,000-93,000, commensurate with experience. The Health and Wellness Director is full-time and as such, receives full-time benefits with PACS.
