LPN (Full Time PMs)

About the Role

Responsible for providing direct nursing care to residents and supervising the day-to-day nursing activities performed by nursing assistants. Supervision must align with current Federal, State, and local standards, guidelines, regulations, and facility policies to ensure the highest degree of quality care is maintained at all times.

Qualifications

• Current, unencumbered license to practice as a RN or LPN in Illinois and/or Wisconsin.
• Current CPR certificate or certification within two weeks of employment.
• Knowledgeable of nursing and medical practices and procedures.
• Demonstrates interest and/or knowledge in care of the aged.
• Able to read, write, and speak English clearly and understandably.
• Ability to make independent decisions when circumstances warrant.
• Tactful in dealing with residents, family members, visitors, personnel, government agencies, and the public.
• Leadership and supervisory ability with a willingness to work harmoniously with others.
• Personable, enthusiastic, and resourceful.

• Able to move intermittently throughout the workday.
• Able to cope with mental and emotional stress of the position.
• Possess adequate sight and hearing (or use prosthetics) to perform job requirements.
• Able to function independently, with flexibility, personal integrity, and work effectively with residents, families, personnel, and support agencies.
• Good general health and emotional stability.
• Able to work with ill, disabled, elderly, emotionally upset, and occasionally hostile individuals.
• May be required to lift, push, pull, or move equipment, supplies, and residents in excess of 50 pounds.
• Subject to exposure to infectious waste, diseases (including HIV and Hepatitis B viruses), and other conditions.

Responsibilities

• Accept and support assignments as directed, which may include different units, residents, workloads, and multiple tasks.
• Plan, develop, organize, implement, evaluate, and direct nursing staff for a respective unit.
• Ensure all nursing procedures and protocols are followed according to established policies.
• Supervise and instruct nursing staff on job-related topics to promote knowledge and compliance.
• Ensure staff adhere to departmental policies, procedures, and dress code.
• Assist in developing individualized care plans for residents and identify their needs.
• Attend and actively participate in resident care plan conferences.
• Report resident or family concerns and complaints to the Administrator.
• Counsel or discipline nursing personnel as necessary.
• Conduct daily rounds to ensure staff perform duties and follow procedures; perform physical rounds with residents daily.
• Make rounds with physicians as needed.
• Maintain a warm, positive, and calm environment for residents and staff.
• Attend continuing education programs.
• Recommend equipment and supply needs to the Director of Nursing.
• Order medications and treatments within budgetary guidelines.
• Utilize PCC 24-hour summaries for resident care recommendations and concerns.
• Complete accident/incident reports thoroughly.
• Encourage physicians to review treatment plans, record orders, and sign progress notes.
• Chart nurses’ notes descriptively to reflect care provided and resident responses.
• Complete required assessments upon resident admission, transfer, or discharge.
• Receive and record physician telephone orders on appropriate forms.
• Record and forward new or changed diet orders to the Dietary Department.
• Perform routine charting duties according to established policies.
• Complete admission, discharge, and transfer processes.
• Prepare and administer medications and treatments as ordered by physicians.
• Review medication records for completeness, accuracy, and adherence to policies.
• Administer professional services such as catheterization, tube feedings, suction, wound care, colostomy care, and post-mortem care as required.
• Participate in orienting new residents and family members to the facility.
• Arrange diagnostic and therapeutic services as ordered by physicians.
• Notify attending physician and family of changes in resident conditions.
• Obtain sputum, urine, and other lab tests as ordered.
• Take and record vital signs, blood pressures, and accuchecks as necessary.
• Contact physicians for nursing assessments, medication orders, incident reports, abnormal lab/X-ray results, and other care-related matters.
• Ensure nurses’ notes and monthly summaries align with care plans.
• Attend staff meetings as directed.
• Perform other related duties as assigned.
