Responsibilities

• Assess patient and family's unmet health and social needs
• Provide effective communications to improve health literacy
• Develop a care plan based on mutual goals with patient, family, and provider
• Create an emergency plan, medical summary, and ongoing action plan
• Maintain patient adherence to plan of care and progress toward goals
• Facilitate patient access to appropriate medical and specialty providers
• Communicate with specialists and Primary Care Physician post-discharge for cohesive care plan development
• Attend and actively participate in care coordinator-related training and meeting activities
• Perform regular visits to provide patient and family support and education

Requirements

• Active and unrestricted LPN license in the state of New York
• 10+ years of clinical nursing experience
• Experience in caring for chronic disease patients
• Experience with navigation of local medical and social support systems
• Care Coordination, Case Management, Home Health, Hospital, or Urgent Care experience

Pay

The hourly pay for this role will range from $20.00 to $36.00 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
