Responsibilities
- Cook up and oversee resident discharge planning for the Sub-Acute Unit
- Participate in utilization review and insurance/Medicare reviews
- Issue Medicare cut letters as needed
- Collaborate with interdisciplinary teams on treatment and discharge plans
- Provide social service support to residents and families
- Cook up and coordinate community resources and post-discharge services
- Maintain accurate records and documentation
Qualifications
- Master’s Degree in Social Work OR Active NJ LPN or RN license
- Minimum 4 years of social work or clinical nursing experience, including admissions/discharge planning
- Long-term care or rehab experience preferred
