About the role
Trustbridge Hospice, a part of Empath Health, is seeking a caring and motivated LPN Coordinator to work in their Boca Raton office. The position involves coordinating and dispatching staff visits on the schedule: Monday - Friday 7:00am – 3:30pm.
Responsibilities
- Coordinate with Managers and team members in patient/family direct and indirect care needs.
- Provide quality professional intervention based on protocols and policy regarding the physical, emotional, and psychological needs of patients.
- Interact with management, physicians, and field staff using a teamwork approach.
- Serve as a professional leader in assessing, planning, guiding, and evaluating patient care activity.
- Pledge to make the comfort and care of patients and families the highest priority.
- Consistently provide a high level of personalized, sincere, and engaging service to patients and families at all times.
- Ensure the complete satisfaction of the patient and family.
Requirements
- Current license to practice as a Licensed Practical Nurse in the State of Florida.
- Current Basic Life Support Certification.
- Minimum experience of one year in public health, home nursing care, oncology nursing, medical/surgical nursing, geriatric nursing, or Hospice nursing.
- Understanding of the ability to apply the characteristics of hospice nursing, case management, crisis intervention, communication skills, thanatology skills, and needs.
- Basic computer skills and EMR software experience (a plus).

Pay
