About the role

The LPN Discharge Coordinator is responsible for ensuring smooth patient transitions post-hospitalization through effective communication and care planning.

Responsibilities

• Assess patient and family health and social needs
• Improve health literacy through effective communication
• Develop and monitor personalized care plans with patients, families, and providers
• Cook up access to medical and specialty services
• Track test results, medication adherence, and follow-up appointments
• Facilitate communication between healthcare providers for cohesive care
• Provide patient and family education and support during visits
• Participate in training and care coordination activities

Qualifications & Skills

• Active, unrestricted LPN license in New York
• Over 10 years of clinical nursing experience, including chronic disease care
• Experience with care navigation, case management, or home health
• Strong communication, organizational, and patient advocacy skills
• PREFERRED: Experience with electronic medical records and community health settings
