About the Role

An LPN provides direct and indirect outpatient nursing care under RN supervision and practitioner orders. They prepare and review medical records, implement basic nursing procedures, administer treatments, medications, and immunizations, and perform point-of-care testing. The role involves documenting care, monitoring patient responses, and collaborating with team members to revise care plans.

Responsibilities

  • Prepare and review patient records; follow up on data and communicate abnormal findings.
  • Administer treatments, immunizations, medications, and perform authorized tests.
  • Document interventions, patient responses, and ensure compliance with standards.
  • Coordinate patient discharge plans, follow referrals, and provide patient education.
  • Manage immunization data, ensure vaccine inventory, and maintain VFC program records.
  • Assist in infection control, train new staff, and support emergency patient care.
  • Maintain supplies, schedule patient visits, and process forms.
  • Provide culturally sensitive care, demonstrate technical skills, and adhere to safety protocols.

Requirements

  • Hawaii LPN license and Basic Life Support certification.
  • Minimum 12 months LPN experience; preferred acute care experience.
  • Knowledge of conflict resolution, customer service, teamwork, and communication.
  • Ability to perform detailed tasks, organize effectively, and work within guidelines.

Skills

  • Proficiency in word processing and spreadsheets.
  • Familiarity with immunization protocols (desirable).
