LPN - Clinical I (Meds)

Responsibilities

• Prepare and review patient records, gather patient data, and communicate abnormal findings to RN or practitioners.
• Administer treatments, medications, immunizations, and perform authorized point-of-care tests.
• Document care, monitor patient responses, and collaborate on care plan revisions.
• Assign tasks to unlicensed staff, ensure compliance with policies, and support emergency situations.
• Follow discharge plans, process referrals, and provide patient education and instructions.
• Manage immunization data, administer vaccines per guidelines, and maintain vaccine inventories.
• Ensure compliance with infection control, CDC, federal, and state guidelines.
• Participate in staff orientation and training.
• Maintain supplies, schedule patient care, and assist with clinical procedures within scope.

Requirements

• Hawaii Licensed Practical Nurse license
• 12 months LPN experience
• BLS certification
• Completed approved LPN curriculum

Qualifications

• Additional skills include conflict resolution, communication, and teamwork.
• PREFERRED: acute care experience and proficiency in word processing.
