About the Role
This position involves providing direct patient care as an LPN, collaborating with RNs based on patient acuity and load.
Responsibilities
- Medication administration
- IV therapy (excluding push medications)
- Dressing changes
- Trach and NG tube care
- Ostomy management
- Documenting assessments and care plans after RN initiation
- Communicating with physicians and advanced practice providers (APPs) for orders and care coordination
- Reinforcing patient education
- Supporting discharge instructions
Requirements
- Licensed Practical Nurse (LPN) license from the Illinois Department of Financial and Professional Regulation (IDFPR)
- Basic Life Support (BLS) certification within 30 days of hire (American Heart Association)
- College diploma as an LPN
Skills
- Competence in medication passing, IV therapy, wound care, and patient documentation
- Strong communication skills to coordinate with the healthcare team and educate patients
