About the Role

The licensed practical/vocational nurse (LPN/LVN) delivers patient care under limited supervision, following the nursing processes of assessment, planning, implementation, and evaluation. Responsibilities are carried out in accordance with federal, state, and local laws, as well as facility-specific standards, policies, procedures, guidelines, and regulations.

Responsibilities

• Provides skilled nursing care as ordered on the plan of treatment and nursing care plan, ensuring each shift or visit reflects care rendered per physician’s orders.
• Identifies new problems, changes, or resolutions, and follows through with:
  • Notification of Director/Supervisor.
  • Physician notification to obtain necessary orders for intervention per state regulations.
  • Documentation of changes on clinical notes, supplemental physical orders, and the nursing care plan.
• Observes aseptic technique as required.
• Performs specific treatments and medication administration as outlined in the care plan.
• Functions as a team member under the direction of the registered nurse, with shifts assigned according to the plan of treatment.
• Communicates skilled observations and significant changes in patient status to the Director of Clinical Services and/or Clinical Manager.
• Requests modifications or additional orders based on the patient’s clinical status and nursing judgment, documenting all physician collaboration in the medical record.
• Demonstrates and documents appropriate utilization of available services.
• Participates in agency-mandated activities, such as in-services, as requested by the Agency Director.
• Adheres to MGA policies, procedures, and the employee handbook, ensuring all clinical documentation is completed accurately and submitted timely per agency policy.
• Represents the company professionally in the community and maintains a clean, neat appearance in accordance with the dress code.
• Reports on time and as scheduled to complete work within designated timeframes.

Requirements

• Graduation from an approved certification program, accredited school, or relevant post-graduate education to perform the job as described.
• Holds a valid and unencumbered license to practice as an LPN/LVN in the state where services are provided.
• Professional experience as required by state regulations to perform the job.
• Successful completion of assigned screening/competency testing with a proficiency score of 80% or greater.
• Reliable transportation.
• Provide a copy of current TB results or chest x-ray.
• Provide a copy of current CPR certification as per agency policy.
• Flexibility, organization, and the ability to exercise independent and sound judgment in performing job duties.

Physical Requirements

• Ability to regularly sit, stand, stop, bend, kneel, walk, balance, crouch, crawl, push, pull, and reach with hands and arms.
• Vision and hearing capabilities within normal limits, with or without aid.
• Ability to frequently lift and/or move up to 15 pounds and occasionally lift or move 100 pounds or more, if applicable.
• Ability to perform the anticipated level of physical assistance required to provide safe client care. If the required assistance exceeds the employee’s safe capacity, the supervisor must be notified immediately.

Work Environment

• Work environment is highly variable, as care is generally provided in private homes with unpredictable lighting, acoustics, air quality, temperature, and environmental hazards.
• Occasional exposure to wet and/or humid conditions, household pets/animals, fumes or airborne particles, and toxic or caustic chemicals.
• Frequent travel is required to complete home health visits.
