Long Term Care Functional Screener

Responsibilities

• Build trust with IRIS participants and administer LTC Functional Screens
• Coordinate and document eligibility renewals, provide guidance to maintain funding
• Collaborate with team members and community partners
• Participate in meetings, prepare reports, and testify when needed
• Ensure compliance with documentation standards and strengthen community connections

Qualifications & Skills

• Bachelor's in social work, psychology, nursing, or related field
• At least 1 year of experience with seniors or individuals with disabilities
• Valid LTC FS Certification (or willingness to obtain)
• Bilingual skills are a plus
• Strong organizational, planning, and interpersonal skills
• Proficiency with smartphones, tablets, and computers
• Commitment to participant autonomy and community resources knowledge

Requirements

• Valid driver's license, reliable vehicle, and satisfactory driving record
• Ability to pass background checks and willingness to travel frequently
