About the role
The Long-Term Care Functional Screener is a full-time role primarily based in Milwaukee, WI, involving travel up to 80%. The position supports individuals enrolled in the IRIS Program by conducting annual and as-needed Long-Term Care Functional Screens (LTC FS), assessing their eligibility for publicly funded long-term care services.
Responsibilities
- Build trust with participants
- Coordinate and document LTC FS renewals
- Provide guidance to maintain funding eligibility
- Collaborate with team members and community partners
- Ensure compliance with documentation standards
Requirements
- Bachelor’s degree in social work, psychology, or related fields
- At least one year of experience working with seniors or individuals with disabilities
- Certification in LTC FS preferred or willingness to obtain
- Valid driver’s license and reliable vehicle
- Passing background checks
