About the role
Long-Term Care Functional Screener responsible for administering and coordinating LTC Functional Screens, building relationships with IRIS participants, and supporting eligibility for ongoing funding.
Responsibilities
- Build trusting relationships with IRIS participants
- Administer and coordinate LTC Functional Screens
- Document renewals and provide guidance to maintain funding eligibility
- Collaborate with team members and community partners
- Participate in meetings and prepare reports
- Testify in hearings as needed
- Ensure compliance with documentation standards
- Strengthen community connections
Qualifications
- Bachelor's degree in social work or related field
- Minimum 1 year of experience working with seniors or individuals with disabilities
- Valid LTC FS certification or willingness to obtain
- Bilingual skills a plus
- Strong organizational and communication skills
- Proficiency with technology
- Ability to work independently and collaboratively
