About the role

A compassionate Long-Term Care Functional Screener is responsible for assessing eligibility for publicly funded long-term care services within the IRIS Program.

Responsibilities

  • Build trusting relationships with IRIS participants.
  • Administer and coordinate Long-Term Care Functional Screens (LTC FS) and renewals.
  • Guide participants to maintain funding eligibility.
  • Collaborate with team members and community partners.
  • Participate in staff meetings, prepare reports, and testify as needed.
  • Ensure compliance with documentation standards and strengthen community connections.

Qualifications & Skills

  • Bachelor's degree in social work, psychology, human services, nursing, or related fields.
  • At least 1 year of experience with seniors or individuals with disabilities.
  • Valid LTC FS Certification or willingness to obtain.
  • Bilingual skills are a plus.
  • Strong organizational, planning, and communication skills.
  • Comfortable with technology and community resources.
  • Ability to work independently and collaboratively.

Requirements

  • Valid driver’s license, reliable vehicle, and satisfactory driving record.
  • Pass background checks and willingness to travel frequently, including overnight stays.
