About the role
A compassionate Long-Term Care Functional Screener is responsible for assessing eligibility for publicly funded long-term care services within the IRIS Program.
Responsibilities
- Build trusting relationships with IRIS participants.
- Administer and coordinate Long-Term Care Functional Screens (LTC FS) and renewals.
- Guide participants to maintain funding eligibility.
- Collaborate with team members and community partners.
- Participate in staff meetings, prepare reports, and testify as needed.
- Ensure compliance with documentation standards and strengthen community connections.
Qualifications & Skills
- Bachelor's degree in social work, psychology, human services, nursing, or related fields.
- At least 1 year of experience with seniors or individuals with disabilities.
- Valid LTC FS Certification or willingness to obtain.
- Bilingual skills are a plus.
- Strong organizational, planning, and communication skills.
- Comfortable with technology and community resources.
- Ability to work independently and collaboratively.
Requirements
- Valid driver’s license, reliable vehicle, and satisfactory driving record.
- Pass background checks and willingness to travel frequently, including overnight stays.
