About the role
Long-Term Care Functional Screener responsible for assessing eligibility for publicly funded long-term care services within the IRIS Program.
Responsibilities
- Build trusting relationships with participants
- Conduct and document LTC Functional Screens
- Coordinate renewals and provide guidance to maintain funding eligibility
- Collaborate with team members and community partners
- Participate in meetings and prepare reports
- Testify in reviews and ensure compliance with documentation standards
- Strengthen community connections
Qualifications & Skills
- Bachelor's degree in social work or related field
- Minimum 1 year experience with seniors or individuals with disabilities
- LTC FS Certification (or willingness to obtain)
- Bilingual skills a plus
- Strong organizational skills and comfort with technology
- Commitment to participant autonomy
- Valid driver's license, reliable vehicle, and ability to travel frequently
