POSITION SUMMARY – LMSW Social Worker Provides comprehensive inpatient and outpatient care coordination for high-risk patients by conducting assessments, developing care plans, connecting patients with community, behavioral health, and healthcare resources, and advocating for their needs. Working as part of an interdisciplinary team, the Social Worker evaluates medical necessity, supports discharge planning, coordinates services across the continuum of care, monitors patient progress, and promotes safe, efficient, and cost-effective care for patients of all ages, including those with mental health and complex psychosocial needs.

Responsibilities

• Provides comprehensive inpatient and outpatient care coordination for high-risk patients by conducting assessments, developing care plans, connecting patients with community, behavioral health, and healthcare resources, and advocating for their needs.
• Works as part of an interdisciplinary team to evaluate medical necessity.
• Supports discharge planning.
• Coordinates services across the continuum of care.
• Monitors patient progress.
• Promotes safe, efficient, and cost-effective care for patients of all ages, including those with mental health and complex psychosocial needs.

Qualifications

• Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW).
• Valid, current, full and unrestricted Professional License to practice LMSW or LCSW in any U.S. state, Puerto Rico, or U.S. territory.
• Master’s degree in social work.
• AHA BLS certification.
• 3+ years’ experience in a clinical care setting.
• Experience and knowledge of discharge planning, case management, or utilization review processes.
• Demonstrated knowledge of electronic health record systems.

Pay

Pay Rate: $1600.00 - $2300.00

Schedule

Length of Assignment: 13 weeks. Shift/Hours per Week: Monday - Friday from 8am - 5pm. Callback on weekends/holidays.
