Position Summary

Provide clinical assessment, crisis intervention, care coordination, and discharge planning to help patients and families navigate medical, emotional, and social needs during hospitalization.

• Provide clinical assessment, crisis intervention, care coordination, and discharge planning to help patients and families navigate medical, emotional, and social needs during hospitalization.

Minimum Required Qualifications

• Oregon LMSWMaster’s degree
• BLS
• 2 years recent social work experience in an acute hospital setting
• Crisis intervention experience

Length of Assignment

13 weeks

Shift / Hours Per Week

• 8a-4:30p
• Every other weekend
• Daily call requirements
