About the Role

The Life Services Life Coach provides one-on-one coaching and development for participants in the Life Services program. This role addresses both urgent and long-term member needs to prepare them for employment or education programs and assist with social determinants of health (SDOH).

Responsibilities

  • Complete required assessments by gathering information from the member.
  • Support a positive environment for the member by motivating, recognizing, and engaging participants through proper communication and individualized practices.
  • Maintain required documentation in the case management system and follow all e-manual instructions.
  • Research developments in workforce and career development for individuals in or working to escape poverty.
  • Act as a liaison to social service agencies (SDOH), government agencies, employers, employment, and education programs.
  • Advocate for Life Services participants to access resources.
  • Coordinate logistics for appointments as necessary.
  • Support development of Care Plan and monitor and report on participant status.
  • Arrange and coordinate services with community leaders and partners to maximize utilization of available resources and support groups.
  • Work with Care Managers (CM) to ensure the Care Plan is carried out appropriately.
  • Travel regularly to conduct member, provider, and community-based visits to ensure effective administration of the program.
  • Meet the minimum caseload requirement set by the supervisor.
  • Perform any other job duties as requested.

Requirements

  • Associate’s Degree in Social Work, Human Services, or Nonprofit Management (or equivalent years of relevant work experience) is required.
  • Bachelor’s Degree in Social Work, Human Services Administration, or Nonprofit Management is preferred.
  • A minimum of two (2) years of social service delivery experience is required.
  • Experience working with populations in poverty is preferred.
  • Experience with Motivational Interviewing is preferred.
  • Intermediate proficiency in Microsoft Office Suite (Word, Excel, PowerPoint).
  • Demonstrated coaching and counseling skills.
  • Ability to influence and change individual behavior to help participants achieve success.
  • Demonstrated leadership skills to develop, lead, and motivate participants.
  • Ability to think analytically, identify trends, and take appropriate actions.
  • Demonstrated change management aptitude.
  • Excellent written and verbal communication skills.
  • Ability to work in a fast-paced and constantly changing environment.
  • Ability to develop, prioritize, and accomplish goals.
  • Effective problem-solving skills with attention to detail.
  • Sensitivity to and experience working with individuals in poverty.
  • Ability to separate personal from professional boundaries.

Preferred Certifications

  • Coaching/Mentoring certification.
  • Training/Working with populations in poverty certification.

Licensure

  • Must have a valid driver’s license, vehicle, and verifiable insurance.
  • Employment is conditional pending successful clearance of a driver’s license record check and verified insurance.
  • Employment is conditional pending successful clearance of a criminal background check.

Health Requirements

Influenza vaccination is required annually (October 1 – March 31) as a condition of continued employment. Employees hired during the influenza season will have thirty (30) days from their hire date to complete the vaccination and verify immunization.

Working Conditions

  • This is a mobile position requiring regular travel to different work locations, including homes, offices, or other public settings.
  • Exposure to weather conditions typical of the location.
  • May be required to stand and/or sit for long periods of time.
  • May require travel greater than 50% of the time to perform work duties.
  • Use of general office equipment, such as a telephone, photocopier, fax machine, and personal computer.
  • Flexible hours, including possible evenings and/or weekends to serve member needs.

Pay

$47,400.00 - $76,000.00 annually. Salary is determined by a combination of education, training, experience, scope and complexity of the role, discretion required, and external/internal data. Bonus eligibility is tied to company and individual performance.
