About the Role

The Life Services Life Coach III provides one-on-one coaching and development for participants in the Life Services program, addressing both urgent and long-term member needs. The role focuses on preparing members for employment or education programs and assisting with social determinants of health (SDOH) needs.

Responsibilities

  • Complete required assessments by gathering information from the member.
  • Support a positive environment for the member through motivation, recognition, and engagement using individualized practices.
  • Maintain required documentation in the case management system and follow all e-manual instructions.
  • Research developments in workforce and career development for individuals in or working to escape poverty.
  • Act as a liaison to social service agencies (SDOH), government agencies, employers, employment, and education programs.
  • Advocate for Life Services participants to access resources.
  • Coordinate logistics for appointments as necessary.
  • Support the development of Care Plans, monitor participant status, and report progress.
  • Arrange and coordinate services with community leaders and partners to maximize resource utilization and support groups.
  • Work with Care Managers to ensure the Care Plan is carried out appropriately.
  • Travel regularly to conduct member, provider, and community-based visits to ensure effective program administration.
  • Maintain the minimum caseload set by the supervisor.
  • Complete additional tasks and projects such as data reporting, data analysis, and trend analysis.
  • Serve as a Subject Matter Expert (SME) in a related business need area (e.g., complex populations such as SUD, SMI, IDD, re-entry, maternal health).
  • Perform any other job duties as requested.

Requirements

  • Associate’s degree in Social Work, Human Services, or Nonprofit Management, or equivalent years of relevant work experience (required).
  • Bachelor’s Degree in Social Work, Human Services Administration, or Nonprofit Management (preferred).
  • Four (4) years of social service delivery experience (required).
  • A minimum of three (3) years of Life Services tenure or relatable experience (required).
  • Experience working with populations in poverty (required).
  • Experience with Motivational Interviewing (required).
  • Coaching or other certification related to the position (e.g., social work, chemical dependency, workforce development, re-entry, IDD, mental/behavioral health) (preferred).
  • Training or certification in working with populations in poverty (preferred).
  • Valid driver’s license, vehicle, and verifiable insurance (required). Employment is conditional pending successful clearance of a driver’s license record check and verified insurance.
  • Successful clearance of a criminal background check (required).
  • Annual Influenza vaccination during the season (October 1 – March 31) as a condition of continued employment.

Working Conditions

  • This is a mobile position requiring regular travel to different work locations, including homes, offices, or other public settings.
  • Exposure to weather conditions typical of the location.
  • May require standing or sitting for long periods.
  • Travel greater than 50% of the time may be required to perform work duties.
  • Use of general office equipment such as telephones, photocopiers, fax machines, and personal computers.
  • Flexible hours, including possible evenings and/or weekends to meet member needs.

Pay

$49,050.00 - $78,570.00 annually. Salary is determined by a combination of education, training, experience, scope and complexity of the role, discretion required, and external/internal data. Bonus opportunities may be available based on company and individual performance.
