Job Summary
The Life Services Life Coach III provides one-on-one coaching and development for participants in the Life Services program, addressing urgent, immediate, and long-term needs to prepare members for employment or education programs and assist with social determinants of health (SDOH).
Essential Functions
- Complete required assessments by gathering information from the member.
- Support a positive environment for the member by motivating, recognizing, and engaging participants through proper communication and individualized practices.
- Maintain required documentation in case management system and follow all e-manual instructions.
- Research developments in the fields of workforce and career development for those in and working to get out of poverty.
- Act as a liaison to social service agencies (SDOH), government agencies, employers, employment and education programs.
- Advocate for Life Services participants to access resources.
- Coordinate logistics for appointments as necessary.
- Support development of Care Plan and monitor participant status.
- Arrange and coordinate services with community leaders; partners to maximize utilization of available resources and support groups.
- Work with Case Manager to ensure Care Plan is carried out appropriately.
- Regular travel to conduct member, provider, and community-based visits as needed to ensure effective administration of the program.
Required
- Associate's degree in Social Work, Human Services, or Nonprofit Management or equivalent years of relevant work experience.
- Bachelor’s Degree in Social Work, Human Services Administration, or non-profit Management preferred.
- Four (4) years of social service delivery experience.
- A minimum of three (3) years of Life Services tenure or relatable experience.
- Experience working with populations in poverty.
- Experience with Motivational Interviewing.
Education and Experience
- Intermediate proficiency in Microsoft Office Suite to include Word, Excel, and PowerPoint.
- Demonstrated coaching and counseling skills.
- Ability to influence and change individual behavior and assist individuals to achieve a high level of success and productiveness.
- Demonstrated leadership skills to effectively develop, lead, inspire, and motivate Life Services participants.
- Ability to think analytically to identify qualitative or quantitative trends and take appropriate actions.
- Demonstrated change management aptitude and ability.
- Excellent written and verbal communication skills.
- Ability to work in a fast-paced environment.
- Adaptable to a constantly changing environment.

Competencies, Knowledge and Skills
- Licenses and Certifications: Coaching or other certification related to position preferred. Examples of licenses or certifications can include but not limited to social work, chemical dependency, workforce development, re-entry, IDD, mental/behavioral health.
- Training/Working with Population in Poverty Certification preferred.
- Valid driver’s license, vehicle, and verifiable insurance.
Employment Conditions
- Conditional pending successful clearance of a driver’s license record check and verified insurance.
- Conditional pending successful clearance of a criminal background check.
- Influenza vaccination is a requirement of this position.
Working Conditions
- This is a mobile position, meaning that regular travel to different work locations, including homes, offices or other public settings, is essential.
- May be required to travel greater than 50% of time to perform work duties.
- Required to use general office equipment, such as a telephone, photocopier, fax machine, and personal computer.
- Flexible hours, including possible evenings and/or weekends as needed to serve the needs of our members.
Compensation
- Compensation Range: $49,050.00 - $78,570.00
Company and Individual Performance Bonus
- Annual proof of Influenza vaccination for designated positions during Influenza season (October 1 – March 31) as a condition of continued employment.
