Licensed Social Worker - Hospice Home Care - Chinese-speaking (Per Diem)

About the role

Provides social work services to patients and families in collaboration with the healthcare team, following organizational policies, and works under general supervision.

Responsibilities

• Assess clients’ psychosocial status, needs, and living conditions using professional observation and interviewing skills.
• Develop and implement social work components of care plans, making referrals to community resources as needed.
• Travel to patients’ homes or facilities to deliver direct service, adapting to varied environments.
• Offer psychosocial support including counseling, crisis intervention, and resource planning.
• Advocate for patients and families, assisting with entitlements and community services.
• Maintain communication through clinical notes and documentation, ensuring quality care.
• Manage caseloads from assessment to discharge, contributing to team treatment plans and goals.
• Educate patients and families on treatment, self-care, and prevention strategies.
• Coordinate discharge planning and utilize community resources; act as a liaison with agencies.
• Participate in professional development, special projects, and other assigned duties.

Qualifications

• Current Licensed Social Worker registration in New York State; LCSW preferred.
• Master’s Degree in Social Work from an accredited program.
• At least one year of social work experience in healthcare setting.
• Bilingual in Chinese (Mandarin or Cantonese) required.
• Valid driver’s license or NYS Non-Driver ID may be needed.
