About the role
As a Social Worker for Caring Hospice, you will provide quality hospice care, end-of-life, educational and grief support services.
Responsibilities
- Conduct hospice assessments and patient visits.
- Provide grief counseling/support.
- Foster open communication with patients, patient’s families, facility partners, staff and other healthcare providers.
- Work effectively on an interdisciplinary team.
- Make funeral arrangements; patient care planning, accessing community resources and discharge planning.
Requirements
- Must be a Licensed Social Worker (LMSW) or a Licensed Clinical Social Worker (LCSW) in the state of MA.
- Minimum of 2 years of hospice experience preferred in hospice or oncology.
- Candidate must have previous experience in conducting comprehensive assessments.
- Ability to work effectively on an interdisciplinary team.
- Prior experience making funeral arrangements; patient care planning, accessing community resources and discharge planning.
- Ability to provide grief counseling/support.
- Ability to foster open communication with patients, patient’s families, facility partners, staff and other healthcare providers.
Qualifications
- Must have reliable transportation, and be able to travel to where the patient resides.

- Strong organizational and problem-solving abilities.
- Ability to work independently and as part of a team.
