Hospice Social Worker (per diem)

Responsibilities

• Assess emotional factors related to terminal illness.
• Assess environmental resources and obstacles to maintain safety.
• Provide emotional support, short‑term crisis intervention, and individual and/or family counseling when indicated.
• Maintain contact with appropriate community agencies and services to promote interagency cooperation and facilitate related referrals.
• Evaluate patient/family response to psychosocial interventions.
• Assess special needs related to cultural diversity, including communication, space, role of family members, and special traditions.
• Perform comprehensive initial psychosocial assessments.
• Assess the patient/family psychosocial status, potential risk of suicide, and/or abuse or neglect.

Qualifications

• Master’s Degree in Social Work.
• Minimum 1 year of medical social work experience; hospice experience preferred.
• Licensed Social Worker (LSW) certification.
• Understanding of hospice philosophy related to families in crisis and end‑of‑life issues.
• Ability to multitask and prioritize workload.
• Willingness to travel.
• Strong verbal and written communication skills.
