Licensed Social Worker - Hospice Home Care

Responsibilities

• Conduct psychosocial assessments for hospice patients.
• Develop and implement person-centered care plans.
• Provide counseling, crisis intervention, and end-of-life support in patients' homes.
• Collaborate with nurses, physicians, chaplains, and caregivers to coordinate community resources and benefits.
• Deliver culturally sensitive care and maintain accurate documentation.
• Advocate for patients to ensure dignity, comfort, and emotional well-being throughout the hospice journey.

Qualifications & Skills

• Licensed Social Worker (LSW) in good standing.
• Strong communication, assessment, and counseling skills.
• Ability to work collaboratively in a multidisciplinary team.
