Responsibilities

• Ensure culturally sensitive care, accurate documentation, and patient advocacy.

Qualifications & Skills

• Licensed Social Worker certification.
• Strong interpersonal and communication skills.
• Ability to work independently and as part of a multidisciplinary team.
• Experience in hospice, palliative care, or related fields preferred.
