Licensed Social Worker - Hospice Home Care
About the role
Provides social work services to patients and families in collaboration with the healthcare team, ensuring adherence to policies. Works under general supervision, assessing clients' psychosocial status, needs, and living conditions through observations and interviews. Establishes patient/family care plans aligned with mutual goals and makes appropriate community referrals. Travels to patients’ homes or facilities to deliver direct services, including counseling, resource planning, and crisis intervention. Offers advocacy, assists with entitlements, and responds to emergent needs. Maintains communication through clinical notes and manages a caseload from assessment to discharge, supporting team understanding of social and emotional factors affecting health. Participates in developing and revising treatment plans, coordinating care, and educating patients and families on self-care and prevention. Utilizes community resources, participates in discharge planning, and engages in professional growth activities. Handles transportation of supplies and acts as a resource for staff and special projects.
Qualifications
- Current NYS license as a Licensed Social Worker (LMSW or LCSW preferred)
- Master's Degree in Social Work from an accredited program
- Minimum one year of healthcare social work experience
- Valid driver’s license or NYS Non-Driver ID (as required)
- Bilingual skills may be required based on operational needs
Pay
USD $70,200 – $87,700 annually
