Responsibilities
- Assesses the psychosocial status of patients and families/caregivers related to the patient's medical condition and environment and communicates findings to the registered nurse and other members of the interdisciplinary group
- Conducts assessments of patients' needs and develops care plans
- Provides individual and group counseling to patients and their families
- Collaborates with healthcare professionals to ensure the best possible care for patients
- Provides information and referral services for patients and families/caregivers regarding practical and environmental needs
- Provides information to patients and families/caregivers and community agencies
- Serves as a liaison between patients and families/caregivers and community agencies
- Develops systems to improve patient outcomes
- Conducts research to identify best practices in social work
- Provides education and training to staff on social work topics
- Maintains accurate records of patient care
Requirements
- Bachelor's or Master's degree in Social Work or related field
- Experience working with adults and elderly populations
- Experience working with hospice, home health, dementia care, and developmental disabilities
- Driver's license and car insurance
- Strong communication skills
- Ability to work independently as well as part of a team
- LCSW or LMSW License
Pay
Hourly Rate: $70 - $75 an hour
