Social Worker (Home Health Per Diem )

• Contributes to the effectiveness of the department/Agency by remaining within the acceptable standard of the Agency's Attendance/Tardiness Control Policy.
• Provides direct social work services in the home which meet Agency and professional standards.
• Meets Agency standards of competency.
• Accurately documents care provided and maintains patient records according to Agency policies.
• Assists with Agency and community programs/committees.

Qualifications

• Master's Degree in Social Work
• Current Social Work License in the State of New Jersey, LCSW preferred
• 1 year of post graduate experience in a health setting
