Licensed Social Worker Ambulatory Kailua Kona - Full-Time Day Shift
About the Role
Provides social work services including assessment, care planning, and case coordination to patients and their families. Offers consultation to staff and other healthcare providers, initiates social work plans of care, and serves as a liaison between Kaiser Permanente and the community to integrate services and coordinate care.
Responsibilities
- Prepares and evaluates assessments, develops and implements social work service plans.
- Provides psychosocial assessment, diagnostic impressions, treatment for individuals, couples, families, and groups, and evaluates practice effectiveness.
- Applies knowledge base, theoretical concepts, and functional skills to effect change in human behavior, emotional responses, and social conditions.
- Performs case management, coordination, intervention, and monitoring of social work service plans.
- Provides information, resource identification, referral services, mediation, advocacy, and education.
- Coordinates services and ensures appropriate patient placement.
- Assists with administration, development, implementation, and evaluation of social work programs and policies.
- Conducts social work consultation and resource development.
- Participates in development activities to stay current with trends in medical social work.
- Engages in performance improvement and implements Quality Improvement processes.
- Assists with the development and implementation of the department’s annual work-plan.
- Performs clinical record audits and maintains educational records.
- Conducts research on social work programs, systems, and policies.
- Documents according to departmental standards, Federal, and State regulations.
- Participates in staff meetings, in-services, committees, and orientation of new personnel.
Requirements
- Master’s degree in social work.
- Licensed Clinical Social Worker (Hawaii) or Licensed Social Worker (Hawaii).
- Basic Life Support certification from the American Heart Association required at hire.
Preferred Qualifications
- Minimum one year of social work experience.
- Healthcare experience working with medical diagnoses.
- Demonstrated ability in networking with community agencies.
Pay
$77,100 - $99,770 per year. Kaiser Permanente strives to offer a market-competitive total rewards package and is committed to pay equity and transparency. The posted pay range is based on possible base salaries for the role. Actual base pay determined at offer will be based on labor market data, internal alignment, and a candidate's years of relevant work experience, education, certifications, skills, and geographic location.
Schedule
- Full-time, 40 hours per week.
- Shift: Day, Monday through Friday, 08:00 AM - 05:00 PM.
- Travel: Yes, 50% of the time.
Primary work location: Kona Medical Offices I, Kailua Kona, Hawaii. Worker location must align with Kaiser Permanente's Authorized States policy.
