Licensed Practical Nurse (Residential Services)

About the role

We are seeking a dedicated Licensed Practical Nurse (LPN) to provide compassionate, person-centered healthcare in residential and day programs. Under the supervision of a Registered Nurse, the LPN assists with medical oversight, direct nursing care, medication management, and health assessments.

Responsibilities

• Support individuals with daily health needs
• Coordinate medical appointments
• Maintain accurate medical records
• Promote infection control and safety practices
• Adhere to regulatory standards and confidentiality requirements
• Work as part of a team to make a meaningful impact in the community

Requirements

• Valid New York State LPN license
• Ability to support individuals with developmental disabilities
• Strong communication skills
• Must obtain CPR and First Aid certifications
