Licensed Practical Nurse - Primary Care Resource Team

Role Overview: The Licensed Practical Nurse (LPN) provides patient care under the supervision of RNs or other designated staff, ensuring adherence to patient care plans, policies, and relationship-based care principles. Key Responsibilities: Collect patient data through observation, communicate findings to support assessment and care planning, deliver care delegated by RNs or providers, and participate in team activities, including quality improvement projects. Qualifications & Skills: Graduate of an accredited practical nursing program or licensed after a defined period in a nursing program; preferred recent LPN experience; excellent verbal and written communication; teamwork skills; computer proficiency; adaptability to unpredictable situations; and current LPN license with BLS certification. Additional Requirements: Ability to work daytime hours with flexibility, and potentially participate in weekend shifts. Compensation & Benefits: Hourly rate between $25.21 and $36.82; full-time, benefits-eligible position with scheduled shifts Monday-Friday, extended hours as needed, and at least one weekend per month.
