Licensed Practical Nurse - Primary Care Team

Role Overview The licensed practical nurse (LPN) provides direct patient care under supervision, assisting with assessments, data collection, and care planning. They deliver care aligned with organizational policies and patient care plans, communicating effectively with patients, families, and the healthcare team. The LPN may participate in quality improvement projects and mentoring. Key Responsibilities Deliver nursing care to assigned patients based on established care plans.Collect and communicate patient data to support assessments and care decisions.Collaborate with team members and accept delegation from RNs and providers.Participate in quality improvement initiatives and team activities.Mentor and instruct colleagues as needed. Qualifications & Skills Graduate of an accredited practical nursing program or passed LPN Boards; recent LPN experience preferred.Current LPN license and Basic Life Support certification.Excellent verbal and written communication skills.Experience working collaboratively in a team environment.Proficiency in computer use; prior EMR experience preferred.Flexibility to work daytime hours and adapt to unpredictable situations. Additional Requirements Ability to work at any designated site within the organization.Possibility of working some weekends and extended hours as needed.
