Licensed Practical Nurse - Primary Care Team

Responsibilities

• Provide nursing care to assigned patients under supervision of RN or other supervisor.
• Collect patient data through observation and communicate findings to support assessment and care planning.
• Deliver care aligned with patient care plans, organizational policies, and relationship-based care principles.
• Participate as a team member, accept delegation, and contribute to quality improvement projects.
• Mentor, instruct, and communicate effectively with patients and staff.

Qualifications & Skills

• Graduate of an accredited practical nursing program or passed LPN Boards.
• One year of recent LPN experience preferred.
• Excellent verbal and written communication skills.
• Experience in team environments and proficiency with electronic medical records.
• Current LPN license, with BLS certification as required by state.
• Flexibility to adapt to changing situations and work hours.
