Licensed Practical Nurse - Primary Care Resource Team - LPN

About the role

The Licensed Practical Nurse (LPN) on the Primary Care Resource Team is responsible for providing direct patient care under the supervision of a Registered Nurse. This position supports the delivery of high-quality healthcare services in a primary care setting.

Responsibilities

• Assess patients' health needs and develop care plans accordingly.
• Administer medications and treatments as prescribed by physicians.
• Monitor patients' vital signs and report any changes to supervising RNs.
• Document patient progress and treatment outcomes accurately.
• Collaborate with other healthcare professionals to ensure comprehensive patient care.

Requirements

• Current licensure as an LPN in Minnesota.
• Bachelor’s degree in Nursing preferred, but not required.
• Experience in a primary care setting is beneficial.
• Strong communication and interpersonal skills.

Qualifications

• High school diploma or equivalent.
• Completion of an accredited LPN program.
• Passing scores on the NCLEX-PN exam.
