Location & Schedule

Spencerport, NY 14559 • Full-Time, 40 hours/week • Monday–Friday 7:00 am–3:30 pm

Pay

$28.73–$32.83 per hour

Responsibilities

  • Deliver compassionate, person-centered health care to individuals in residential settings
  • Administer medications and treatments as prescribed; monitor health status and report changes to the RN
  • Provide personal care, support daily living routines, and promote independence
  • Coordinate and accompany individuals to medical appointments; ensure follow-up care is carried out
  • Assist the RN with developing nursing care plans, health-related goals, and staff training
  • Respond to emergencies and behavioral incidents, utilizing SCIP-R strategies
  • Act as shift leader when assigned and collaborate with interdisciplinary teams
  • Maintain accurate documentation in compliance with agency and regulatory requirements
  • Support a clean, safe, and welcoming home environment

Requirements

  • Current New York State Licensed Practical Nurse (LPN) license in good standing
  • CPR and First Aid certification (or ability to obtain); SCIP-R Level II certification or willingness to complete training
  • Valid NYS driver’s license and ability to meet Lifetime Assistance’s Vehicle Operator Requirements
  • Strong communication, problem-solving, and decision-making skills; ability to follow medical directions and adapt to changing care needs
  • Ability to lift up to 35 pounds, respond to emergencies, and perform frequent physical activity
