Part-Time, 16 hours/week – Monday & Friday 6 am–2 pm
Pay
Starting wage: $28.73–$32.83 per hour

Responsibilities
- Deliver compassionate, person-centered health care to individuals in residential settings
- Administer medications and treatments as prescribed; monitor the patient’s health status and report any changes to the RN
- Provide personal care, support daily living routines, and promote independence
- Coordinate and accompany individuals to medical appointments; ensure follow-up care is carried out
- Assist the RN with developing nursing care plans, health-related goals, and staff training
- Respond to emergencies and behavioral incidents, utilizing SCIP-R strategies
- Act as shift leader when assigned and collaborate with interdisciplinary teams
- Maintain accurate documentation in compliance with agency and regulatory requirements
- Support a clean, safe, and welcoming home environment
Requirements
- Current New York State Licensed Practical Nurse (LPN) license in good standing
- CPR and First Aid certification (or ability to obtain); SCIP-R Level II certification or willingness to complete training
- Valid NYS driver’s license and ability to meet Lifetime Assistance’s Vehicle Operator Requirements
- Strong communication, problem-solving, and decision-making skills; ability to follow medical directions and adapt to changing care needs
- Ability to lift up to 35 pounds, respond to emergencies, and perform frequent physical activity
