Location: Brockport, NY, 14420 | Split between two sites in Brockport, NY
Department: Clinical
Reports To: Residence Manager
Employment Type: Full-Time, 40 hours/week
Schedule: Monday, Tuesday, Friday 8:00 am–4:30 pm; Wednesday & Thursday 7:00 am–3:30 pm
Starting Wage: $28.73–$32.83 per hour

Your Core Responsibilities
- Deliver compassionate, person-centered health care to individuals in residential settings.
- Administer medications and treatments as prescribed; monitor health status and report changes to the RN.
- Provide personal care, support daily living routines, and promote independence.
- Coordinate and accompany individuals to medical appointments; ensure follow-up care is carried out.
- Assist the RN with developing nursing care plans, health-related goals, and staff training.
- Respond to emergencies and behavioral incidents, utilizing SCIP-R strategies.
- Act as shift leader when assigned and collaborate with interdisciplinary teams.
- Maintain accurate documentation in compliance with agency and regulatory requirements.
- Support a clean, safe, and welcoming home environment.
What You Bring
- Education & Licensure: Current New York State Licensed Practical Nurse (LPN) license in good standing.
- Certifications: CPR and First Aid (or ability to obtain); SCIP-R Level II certification or willingness to complete training.
- Requirements: Valid NYS driver’s license and ability to meet Lifetime Assistance’s Vehicle Operator Requirements.
- Experience & Skills: Strong communication, problem-solving, and decision-making skills; ability to follow medical directions and adapt to changing care needs.
- Physical Requirements: Ability to lift up to 35 pounds, respond to emergencies, and perform frequent physical activity.
