Licensed Practical Nurse (LPN), ElderONE (Full Time, Days)

About the role

The Licensed Practical Nurse is responsible for providing direct patient care, usually under the direction of a Registered Nurse, in accordance with established plans.

Responsibilities

• Counsels patients and families on health maintenance, nutrition, and hygiene.
• Assists in the implementation of the patient’s treatment plan under the direction of the RN.
• Provides routine nursing services for patients as directed by the RN.
• Completes required documentation in an accurate and timely manner.
• Attends and participates in department/facility meetings as required.
• Maintains clinical competency by pursuing continuing education opportunities.
• Performs other duties as assigned.

Requirements

• Licensed Practical Nurse (LPN) license in the State of New York.
• BLS Certification from the American Heart Association (AHA).

Skills

• Strong communication, customer service, and interpersonal skills.
• One year of experience working with a frail or elderly population preferred.

Qualifications

• Graduate from an accredited School of Practical Nursing.

Benefits

Typically, travel is limited to one to two locations per day, with up to four sites in Rochester, NY and one in Newark, NY. Mileage reimbursement is provided.

Pay

$28.51 - $34.68 per hour

Schedule

Monday - Friday, 8:30am to 4:30pm with occasional weekends.
