Licensed Practical Nurse (LPN) - ElderONE (Part-Time, Day/Evening)

Responsibilities

• Counsels and educates patients and families about health maintenance, nutrition, and self-care.
• Assists in the implementation of the patient’s treatment plan under the direction of the RN.
• Provides routine nursing services for patients as directed by the RN.
• Completes required documentation in an accurate and timely manner.
• Attends and participates in department/facility meetings as required.
• Maintains clinical competency by pursuing continuing education opportunities.
• Performs other duties as assigned.

Requirements

• Licensure / Certification: Graduate from an accredited School of Practical Nursing. Current LPN licensure in State of New York. Basic Life Support (BLS) Certification from the American Heart Association (AHA).
• Skills: 1 year experience working with a frail or elderly population preferred.

Physical Requirements

Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.
