Licensed Practical Nurse (LPN), ElderONE (Per Diem)

Responsibilities

• Counsels and educates patients and their families regarding health maintenance, nutrition, and self-care.
• Assists in the implementation of the patient’s treatment plan under the direction of the RN.
• Provides routine nursing services for patients as directed by the RN.
• Completes required documentation in an accurate and timely manner.
• Attends and participates in department/facility meetings as required.
• Maintains clinical competency by pursuing continuing education opportunities.
• Performs other duties as assigned.

Requirements

• Licensure / Certification: Graduate from an accredited School of Practical Nursing, Current LPN licensure in State of New York, BLS - Basic Life Support - American Heart Association (AHA), American Heart Association (AHA), LPN - Licensed Practical Nurse - New York State Education Department (NYSED), New York State Education Department (NYSED), New York State Education Department (NYSED).
• Skills: Strong communication, customer service, and interpersonal skills highly desired. 1 year of experience working with a frail or elderly population preferred.

Qualifications

• Physical Requirements: M - Medium Work - Exerting 20 to 50 pounds of force occasionally, and/or 10 to 25 pounds of force frequently, and/or greater than negligible up to 10 pounds of force constantly to move objects; Requires frequent walking, standing or squatting.

Benefits

This position has a designated home base facility and may require occasional travel to additional locations. Mileage reimbursement is provided.

Pay

The listed base pay range is a good faith representation of current potential base pay for a successful full time applicant. It may be modified in the future and eligible for additional pay components.
