Licensed Practical Nurse (LPN) Night Shift

About the role

Join the team as a Licensed Practical Nurse (LPN) at our Lexington Community Hospice House. This full‑time night‑shift position (7 pm – 7 am) focuses on creating meaningful patient experiences by providing hospice care under the direction of a registered nurse. You will conduct patient visits, deliver routine care, and communicate with the care team, patients, and families while having access to supportive leadership, professional development, and advancement opportunities.

Responsibilities

• Conduct patient visits and provide routine hospice care.
• Work under the direction and supervision of a registered nurse.
• Communicate effectively with the care team, patients, and families.
• Collaborate with interdisciplinary team members to ensure comprehensive care.

Qualifications

• A heart to serve patients and families and a passion for providing the best possible care.
• Graduate of an accredited school of nursing with a current state license as a Licensed Practical Nurse.
• 2+ years of nursing experience in an acute‑care setting, or equivalent experience (hospice experience preferred).
• Reliable transportation.
• Ability to sit, stand, bend, move intermittently and lift 80‑100 lb, bearing the weight of an average adult.
