Licensed Practical Nurse, LPN
About the role
Join our Aegis Family as a Home Health and Hospice Licensed Practical Nurse (LPN) where you will play a crucial role ensuring our patients receive exceptional care. Our LPN's are responsible for verifying and executing the plan of care as prescribed by physicians and under the guidance of a Registered Nurse. This includes providing essential nursing treatments, promptly addressing patient pain, assisting with activities of daily living, and educating both patients and caregivers on self-care techniques. They also handle medication administration, ensure the availability of necessary supplies, maintain accurate patient records, and consistently liaise with supervising RNs or physicians on patient assessments and any changes in conditions. Moreover, they actively contribute to team meetings to ensure cohesive patient care coordination.
Responsibilities
- Verify and execute the plan of care as prescribed by physicians and under the guidance of a Registered Nurse.
- Provide essential nursing treatments, promptly address patient pain, assist with activities of daily living, and educate patients and caregivers on self-care techniques.
- Handle medication administration, ensure the availability of necessary supplies, maintain accurate patient records, and consistently liaise with supervising RNs or physicians on patient assessments and any changes in conditions.
- Actively contribute to team meetings to ensure cohesive patient care coordination.
Requirements
- Current Licensed Practical Nursing License in state of practice.
- 1+ years of Home Health & Hospice Experience preferred.
- Ability to work independently, be self-directed, and flexible.
- Ability to use a full range of motion and ability to lift 35lbs.
- Effective verbal & written communication & time management skills.
- Valid Driver's License (clean record) & reliable transportation.
- Proof of current auto insurance & registration.
- Valid AZ Fingerprint Clearance Card (we can help).
- Current CPR Certification (we can help) & Negative TB Test (we can test).

Pay
$33-$37 per hour DOE
Schedule
M-F 8am to 5pm
