About the role
Licensed Practical Nurse (LPN) providing skilled nursing care in various healthcare facilities within Hampton Roads.
Responsibilities
- Administer medications and treatments within the LPN scope of practice.
- Collect, document, and monitor patient information and conditions.
- Perform wound care and educate patients on care plans.
- Assist with activities of daily living and coordinate care with healthcare team members.
- Report changes in patient status and ensure quality care delivery.
Qualifications & Skills
- Active LPN license and minimum 1 year of facility-based experience.
- Current BLS/CPR certification and recent PPD screening.
- Ability to meet physical requirements of the role, with or without accommodations.
